Watch your vitamins!

Data Synthesis When all low- and high-bias risk trials of antioxidant supplements were pooled together there was no significant effect on mortality (RR, 1.02; 95% CI, 0.98-1.06). Multivariate meta-regression analyses showed that low-bias risk trials (RR, 1.16; 95% CI, 1.05-1.29) and selenium (RR, 0.998; 95% CI, 0.997-0.9995) were significantly associated with mortality. In 47 low-bias trials with 180 938 participants, the antioxidant supplements significantly increased mortality (RR, 1.05; 95% CI, 1.02-1.08). In low-bias risk trials, after exclusion of selenium trials, beta carotene (RR, 1.07; 95% CI, 1.02-1.11), vitamin A (RR, 1.16; 95% CI, 1.10-1.24), and vitamin E (RR, 1.04; 95% CI, 1.01-1.07), singly or combined, significantly increased mortality. Vitamin C and selenium had no significant effect on mortality.
Conclusions Treatment with beta carotene, vitamin A, and vitamin E may increase mortality. The potential roles of vitamin C and selenium on mortality need further study.

Are we now wasting our money somewhere else?

Deciding what to eat for dinner can be mind-bending. How do we keep track of the ever-evolving recommendations for what to put on, and leave off, the plate? Red meat might cause cancer! But don't replace it with tofu—soy concoctions might be carcinogenic, too! Don't even try to figure out where carbs stand this week. And the verdict on coffee, chocolate, and alcohol changes faster than you can order a mocha martini.
Vitamins—with their promise to bridge the gap between the nutrients our bodies need and those they get—have always seemed reassuringly simple: Just pop a multivitamin and let your body soak in those extra nutrients. But not any longer. During the past few years, study after study has raised doubts about what, if any, good vitamins actually do a body. They could even pose some real medical risks.
Half of all American adults take some sort of nutritional supplement. But research on a wide variety of patient populations and medical conditions has failed to find much evidence that multivitamins, the most commonly used of the lot, prevent major chronic diseases in healthy people. The most recent knockcame this spring, when a study of more than 160,000 post-menopausal women, published in the Archives of Internal Medicine, found that the all-in-one pills did not prevent cancer, heart attacks, or strokes and did not reduce overall mortality.
Individual vitamins and minerals haven't fared much better under scientific scrutinywith research debunking some of the reputed benefits of vitamin B6, calcium, niacin, and others. In 2006, the National Institutes of Health convened an independent panel of experts to evaluate the evidence that vitamins could prevent chronic disease. The scientists ultimately issued a report stating that studies "do not provide strong evidence for beneficial health-related effects of supplements taken singly, in pairs, or in combinations."
The news on antioxidants, the darlings of the vitamin menagerie, is even more troubling. These compounds, which include vitamins A, C, and E, selenium, beta carotene, and folate, fight free radicals, unstable compounds thought to damage cells and contribute to aging. But not only do antioxidant supplements fail toprotect against heart disease, stroke, and cancer; they actually increase the risk of death, according to a 2007 analysis of research on more than 232,000 people, published in the Journal of the American Medical Association, as well as other studies.
Exactly why they might increase mortality is unclear, but doctors at prominent research institutions—including New York's Memorial Sloan-Kettering Cancer Center and Seattle's Fred Hutchinson Cancer Research Center—have highlighted some unsettling connections between supplemental antioxidants and an increased risk of a variety of cancers. Popping certain kinds of antioxidant pills can feed latent cancers growing in the body, for instance, and reduce the effectiveness of chemotherapy. These observations make a certain intuitive sense, since vitamins and minerals play an important role in the replication of healthy cells—why shouldn't they be doing the same for cancerous cells? (Feeding mice a diet poor in antioxidants, on the other hand, can actually help shrink their brain tumors.) Scientists are also beginning to suspect that the body may actually need free radicals—which help kill cancer cells, ensure optimal immune function, and regulate blood sugar, among other things—so we shouldn't necessarily be mopping them all up.
The list of worrisome findings goes on, but it doesn't seem to have put a dent in the $25 billion supplement industry. Sales are not only robust but rising in the United States. Doctors still recommend multivitamins as part of basic preventative care. Despite the demonstrated risk, as many as 80 percent of cancer survivors swallow a daily dose, according to a study published in the Journal of Clinical Oncology in 2008.
Vitamins have a powerful psychological hold over us. As precautionary health measures go, supplements are easy. Compare the two seconds required to swallow a pill with the constant vigilance necessary to exercise and eat right. And the fact that vitamins are available without a prescription makes them seem safe—even though it probably makes them less so, since they're not regulated by the FDA as drugs, and manufacturers are not required to prove that they're effective at treating disease. *
But the risk-benefit calculus has changed. We know more about the risks, and it's clear that there's also less potential benefit. During the early 20th century, diseases like scurvy and rickets were common until researchers began to isolate compounds in food—which became known as vitamins—that could altogether cure these ailments. It must have been remarkable to see devastating diseases alleviated with common foodstuffs.
During an era when many people legitimately had nutritional deficiencies, placing your bets on a multi might have been reasonable. But today, of course, actual deficiencies are much less common. Our salt, milk, flour, juice, cereal, and more are all fortified with extra nutrients, and a 2009 study published in the Archives of Pediatrics and Adolescent Medicine suggests that most of the kids who end up taking vitamins in the United States today don't actually need them.
If vitamins are useful for anything, it's probably for tapping into our old friend the placebo effect.In a 2008 survey, 38 percent of doctors confessed to recommending vitamins because they believed the pills could promote health purely through the power of positive expectations. Consider a famous 1975 study designed to probe whether vitamin C supplements alleviated colds better than a placebo, an inert lactose tabletIt turned out that it didn't matter much which pill the subjects were actually taking.What mattered was what they thought they were getting: Those who believed they were taking vitamin C had fewer and milder cases of the sniffles than those who believed they were just swallowing lactose.That would be reason enough to pop a supplement—there are worse things than deceiving yourself into better health—if it weren't for the emerging evidence that the pills might be capable of causing real harm.
That's not to say that vitamins aren't important. Vitamins are critical to all sorts of bodily functions, and we have to get them through diet because our bodies can't make them on their own. The Office of Dietary Supplements at the NIH recommends that we get certain levels of a variety of kinds of vitamins, and that recommendation is sound. But encouraging us to get a complete suite of vitamins is not the same as suggesting that we get them by popping a pill.
In fact, the reports littering the ODS site seem to converge upon the same point: There is some good news for supplements, but it's extremely limited.The 2006 NIH panel, for instance, concluded that postmenopausal women should probably take calcium and vitamin D to safeguard their bones; that pregnant women should keep taking folate; and that adults with age-related macular degeneration, an eye disease, should take a combination of antioxidants and zinc. But beyond that, the panel's strongest recommendation was that scientists conduct further research on the risks and benefits of vitamins.For every study that turns up disconcerting vitamin side effects, there seem to be two more that conclude that we simply don't know enough yet about supplements to make evidence-based recommendations.
Until we do, we should stop treating supplements like health candy and more like prescription meds, to be used only when there's a demonstrated need. Doctors should create individualized regimes, tailored to a particular patient's deficiencies. As for the rest of us, we can put the pills back on the shelf and save our cash for one of those martinis.

Antioxidants do NOT work

Few medical remedies have a more sterling reputation than that assortment of foods, pills, and general life maneuvers known collectively as "antioxidants." At last, here's something that promises better heart health, improved immunity, a pellucid complexion as well as relief against cancer, arthritis, and the blahs—and it's all-natural! What's not to like?
Well, there is a wee small problem in our ongoing anti-oxidize-athon: As it turns out, we have no evidence that antioxidants are beneficial in humans. (Though if you're a Sprague-Dawley rat, there's hope.) In fact, as Emily Anthes wrote last year in Slate, the best available data demonstrate that antioxidants are bad for you—so long as you count an increased risk of death as "bad."
But, hey, who ever let a little evidence stand in the way of a good time? Especially in this case, when the charge toward lifestyle legitimacy has been led by willowy celebrities with karmic equipoise, ably supported by the Four Horsemen of the Alternative: Drs. Weil, Oz, Null, and Chopra. The seduction of this confederacy (sex! doctors! pills!) is immense; to appreciate its power, one need only consider the pomegranate. Once a rare fruit requiring a complicated eating strategy, its derivatives are now stationed on every grocery shelf based on their promise of an antioxidant punch.
The field of antioxidants is further buffeted by the fact that no one really understands much about them, so winning an argument is greatly simplified. (And never mind that the main commercial use of antioxidants is to act as food preservatives, placing them squarely on the axis of toxicity). Their story began in the 1940s when a physician-chemist named Denham Harman set out to determine the biochemical explanation for aging. As a young man, he had worked in the lubricating department of Shell—a place where the problems of a chemical spoilage caused by "free radicals" were well-known. By the mid-50s Harman hit upon the theory that the same free radicals that were cutting into petroleum industry profits could also simply and completely explain the phenomenon of aging. Better yet, he said, their effects could be ameliorated by something called antioxidants.
But first, what exactly is a free radical? Not a fiery Berkeley politico but rather a lonely unpaired electron lost and spinning around an atomic nucleus. Electrons are compelled to travel in pairs; an unpaired electron therefore will greedily grab the next electron it meets, regardless of pedigree or station. DNA, RNA, mitochondria—it's open season. Sometimes the miscegenation does serious damage to the cell, resulting (the theory goes) in a gradual and irreversible deterioration. For us this means wrinkles and sagging and creaking joints.
The role of antioxidants in this schema is simple: They are brought onboard to hook up with the vagrant electrons and prevent damage. Harman's observation was supported by then-novel insights into management of radiation exposure, a large concern in the 1950s. One known effect of radiation was tissue damage that looked to us like aging. Among the treatments that seemed to blunt this effect were antioxidants such as melatonin, Acetylcysteine, and pentoxiphylline. Presto: A star was born. Never mind how meager the slings and arrows of everyday life are compared with radiation from an atom bomb. If it (might) work for gamma rays, imagine what it can do for my tennis elbow.
If only reality would play along. As noted by Anthes, and Michael Specter in his book Denialism, the first clear crack in the façade was the 2007 revelation in JAMA that antioxidant vitamins were not merely useless but harmful. Building on this clinical observation, a German group has developed a plausible scientific explanation of the increased risk. The title of the group's most recent publication, " Extending life span by increasing oxidative stress," pretty much sums up their view: The human cell should toughen up. It can benefit from enduring something harsh like the insult caused by free radicals. This way, the organism is more prepared to fend off the inevitable Big One, be it cancer or a toxic fume or perhaps a bout of cholera. Practice makes perfect.
Whether this line of inquiry turns out to be hokum or the first step toward elucidation of antioxidants' role may take years to sort out. What is evident immediately however is the wishy-washiness of your local doctor throughout the antioxidant debate: We were slow to jump onto the antioxidant bandwagon and are slower still to jump off.
This brand of muddle is standard operating procedure for clinicians; we are eternally trying to appear modern and open to the new, while remaining anchored in a reality-based mindset that demands facts and data. Many patients are put off by such caution and come to view us as fuddy-duddies, self-promoting sticks in the mud, flapping bags of hot air (not to mention greedy bastards who may be invested, literally, in maintenance of the status quo). Or else we are seen as being handcuffed and controlled by the rigid iron of Big Pharma.
With something as popular as antioxidants, which comprise a substantial piece of the $23 billion U.S. vitamin and supplement market, the problem is particularly acute. If we swim against the riptide of popular delusion and warn our patients against them, it's like we're denying the benefits of sunshine and fresh air. Any appeal to "evidence" in support of that view would have little traction with the consumer; double-blinded medical studies are counterbalanced, in America, by a parallel system of peer review in the form of a nonstop confab of health-themed talk shows, print magazines, and blogs.
So when a patient comes in to ask (or tell) me about the curative powers of antioxidants, I am stuck. I can give him a boring recitation of the latest findings, or mention that the National Center for Complementary and Alternative Medicine has declared that "there is limited scientific evidence to support the use of antioxidant supplements to prevent disease." And he'll tell me that the pills worked wonders for his sister-in-law out in Queens. We both sigh and look away. If he goes ahead and starts guzzling pomegranate juice, he doesn't tell me.
This momentary awkwardness exposes a larger and potentially troubling problem: the fragility of that most complicated of all partnerships, the one between doctor and patient. Many are in an ongoing tug-of-war because of a simple difference of opinion: The patient wants to live forever (natch!) and may embrace any harebrained scheme available (cf.: antioxidants); in response, the doctor assumes the chilly pose of the threatened medical ruling class.
Usually, the tug-of-war doesn't amount to much. Most doctor visits turn out to be a big nothing: a bump that isn't; a pain that fades; a rash that will go away. But once in a while, and especially as people descend the hill from middle age, that routine visit finds its way to a miserable, frightening place. Bad news is disclosed; plans must be made. And that's when antioxidants do their real harm. The arguing through the years regarding pomegranate juice and vitamin E and whether to buy a juicer has slowly corroded the mutual confidence that underlies the doctor-patient relationship. And this is a damage that has no quick remedy, real or imagined.

Anrtioxidants

The American Heart Association, along with the Mayo Clinic and Cleveland Clinic, recommend getting antioxidants from whole foods and a wide variety of foods. While it’s always ideal, and usually more beneficial, to get antioxidants or other nutrients directly from real food sources, certain types may also be helpful when consumed in supplement form.
There’s still debate over which antioxidants may offer help preventing or treating diseases when consumed in concentrated dosages. Some research has shown that antioxidants like lutein and glutathione may be beneficial when taken in supplement form — for example, in preventing vision loss, joint problems or diabetes. But other research doesn’t always show the same results and sometimes even that certain supplements like vitamin A or vitamin C may be harmful in high amounts.
So just remember that while they might help you in certain instances, overall it doesn’t seem that consuming supplemental antioxidants help you live longer. That’s where your diet and lifestyle come in. Bottom line: We should never rely on supplements to counteract unhealthy lifestyles and poor nutrition.
If you’re generally healthy and eat a varied diet, you might not benefit much from taking antioxidants supplements. However, if you’re at risk for something like vision loss or heart disease, talk to your doctor about whether the following antioxidant supplements in proper doses (and with a healthy lifestyle) might be helpful:
1. Glutathione
Glutathione is considered the body’s most important antioxidant because it’s found within the cells and helps boost activities of other antioxidants or vitamins. Glutathione is a peptide consisting of three key amino acids that plays several vital roles in the body, including helping with protein use, creation of enzymes, detoxification, digestion of fats and destruction of cancer cells.
2. Quercetin
Derived naturally from foods like berries and leafy greens, quercetin seems to be safe for almost everyone and poses little risks. Most studies have found little to no side effects in people eating nutrient-dense diets high in quercetin or taking supplements by mouth short term.
Amounts up to 500 milligrams taken twice daily for 12 weeks appear to be very safe for helping manage a number of inflammatory health problems, including heart disease and blood vessel problems, allergies, infections, chronic fatigue, and symptoms related to autoimmune disorders like arthritis.
3. Lutein
Lutein has benefits for the eyes, skin, arteries, heart and immune system, although food sources seem to be generally more effective and safer than supplements. Some evidence shows that people who obtain more lutein from their diets experience lower rates of breast, colon, cervical and lung cancers.
4. Vitamin C
Known for improving immunity, vitamin C helps protect against colds, the flu, and potentially cancer, skin and eye problems.
5. Resveratrol
Resveratrol is an active ingredient found in cocoa, red grapes, and dark berries, such as lingonberries, blueberries, mulberries and bilberries. It’s a polyphonic bioflavonoid antioxidant that’s produced by these plants as a response to stress, injury and fungal infection, helping protect the heart, arteries and more.
6. Astaxanthin
Astaxanthin is found in wild-caught salmon and krill and has benefits like reducing age spots, boosting energy levels, supporting joint health and preventing symptoms of ADHD.
7. Selenium
Selenium is a trace mineral found naturally in the soil that also appears in certain foods, and there are even small amounts in water. It supports the adrenal and thyroid glands and helps protect cognition. It may also fight off viruses, defend against heart disease and slow down symptoms correlated with other serious conditions like asthma.
8. Lavender Essential Oil

1. Slow the Effects of Aging by Reducing Free Radical Damage
As described above, the single most important benefit of antioxidants is counteracting free radicals found inside every human body, which are very destructive to things like tissue and cells. Free radicals are responsible for contributing to many health issues and have connections to such diseases as cancer and premature aging of the skin or eyes.
What do free radicals do exactly, and why are they so destructive? The body uses antioxidants to prevent itself from the damage caused by oxygen. Electrons exist in pairs; free radicals are missing an electron. This is their weapon of sorts. They “react” with just about anything they come into contact with, robbing cells and compounds of one of their electrons. This makes the affected cell or compound unable to function and turns some cells into “electron-seeking muggers,” leading to a chain reaction in the body and the proliferation of free radicals. Free radicals then damage DNA, cellular membranes and enzymes.
2. Protect Vision and the Eyes
The antioxidants vitamin C, vitamin E and beta-carotene have all been shown to have positive effects on preventing macular degeneration, or age-related vision loss/blindness. Many foods that provide these nutrients also supply antioxidants called lutein and zeaxanthin, nicknamed the eye vitamins, and found in brightly colored foods like fruits and vegetables — especially leafy greens and types that are deep orange or yellow.

Since antioxidants help prevent damage of tissues and cells caused by free radicals, they’re needed to protect against heart disease and stroke. At this point, the data does not show that all antioxidants are effective in protecting against heart disease, but some, such as vitamin C, do seem to be.
The American Journal of Clinical Nutrition featured a study that found those with high levels of vitamin C in their blood had almost a 50 percent decreased risk of stroke. Countless studies also have found that people who consume highly plant-based diets — loaded with things like fresh veggies, herbs, spices and fruit — have a better chance of living longer and healthier lives with less heart disease. The Department of Preventive Medicine & Public Health at University of Navarra states, “Fruits and vegetables are dietary sources of natural antioxidants and it is generally accepted that antioxidants in these foods are key in explaining the inverse association between fruits and vegetables intake and the risk of developing a cardiovascular event or having elevated levels of cardiovascular risk factors.” (7) However, when it comes to heart health, certain studies have found that using vitamin E or beta-carotene supplements should be “actively discouraged” because of the increase in the risk of heart-related mortality. 5. May Help Decrease Risk of Cancer
Studies have found that high intakes of vitamin A, vitamin C and other antioxidants could help prevent or treat several forms of cancer thanks to their ability to control malignant cells in the body and cause cell cycle arrest and apoptosis (destruction) of cancer cells. Retinoic acid, derived from vitamin A, is one chemical that plays important roles in cell development and differentiation as well as cancer treatment. 
Lung, prostate, breast, ovarian, bladder, oral and skin cancers have been demonstrated to be suppressed by retinoic acid. (9) Another study collected numerous references demonstrating the findings of retinoic acid in protection against melanoma, hepatoma, lung cancer, breast cancer and prostate cancer. Are There Any Antioxidant Precautions or Side Effects?

Just like any other supplement, it doesn’t seem that it’s beneficial or even necessarily safe to consume high doses of antioxidants in supplement form. For example, because during exercise oxygen consumption can increase by a factor of more than 10, taking high doses of antioxidants might interfere with proper exercise recovery. (12) Other research has shown that high-dose antioxidant supplementation may interfere with the cardiovascular benefits of exercise, have negative effects on the body’s natural anti-cancer activities, and affect how the body balances levels of different chemicals and nutrients on its own. When it comes to protection against things like cancer or heart disease, overall the medical literature seems conflicting. Although some studies found a positive relationship between antioxidant supplementation and risk reduction, others have not found such positive effects. (15) To be safe, always follow directions carefully and speak with your doctor if you’re unsure of whether or not a supplement is right for you. And to remain your healthiest into older age, aim to reduce free radical load in your body by practicing things like:
  • avoiding environmental pollutants in water
  • reducing chemical exposure in household and cosmetic products
  • limiting intake of processed and refined foods
  • limiting intake of pesticide and herbicide-laden foods
  • limiting intake of antibiotic and hormone-laden foods
  • avoiding overuse of medications
  • reducing stress
  • getting moderate amounts of exercise
  • using natural, cold-pressed oils (heat oxidizes fats in refined oils)

Final Thoughts on Antioxidants and High Antioxidant Foods

  • Antioxidants inhibit oxidation in the body, also called free radical damage, which is tied to stress.

Antioxidants

While there are many ways to describe what antioxidants do inside the body, one definition of antioxidants is any substance that inhibits oxidation, especially one used to counteract the deterioration of stored food products or removes potentially damaging oxidizing agents in a living organism.
Antioxidants include dozens of food-based substances you may have heard of before, such as carotenoids like beta-carotene, lycopene and vitamin C. These are several examples of antioxidants that inhibit oxidation, or reactions promoted by oxygen, peroxides and/or free radicals. (1) Research suggests that when it comes to longevity and overall health, some of the benefits of consuming antioxidant foods, herbs, teas and supplements include:
  • Slower signs of aging, including of the skin, eyes, tissue, joints, heart and brain
  • Healthier, more youthful, glowing skin
  • Reduced cancer risk
  • Detoxification support
